Yuen Toh
Joint Clinical Director and Lead Pharmacist, Apex PCN
Yuen Toh is the Joint Clinical Director for Apex PCN, where she drives innovative population health management and strengthens collaborative working across the wider Integrated Care System. As the Lead Pharmacist for the PCN pharmacy team, she is passionate about using shared intelligence, safer systems, and cross‑sector teamwork to improve outcomes.
A Lead Partner, Mental Health Champion and Clinical Safety Officer, Yuen also co‑chairs the Connected Culture Forum, championing Equality, Diversity and Inclusion, and is a committed educator across universities, IMG programmes and postgraduate training.

Melanie Harding
Pharmacy Technician, Apex PCN
Melanie Harding is one of Apex PCN’s longest‑serving pharmacy technicians, has been instrumental since the PCN’s inception. She developed and led the shared alert initiative and plays a key role in training new team members. Mel regularly teaches pharmacy students and trainee pharmacists, promoting proactive, inclusive and safety‑focused practice.
Together, Yuen and Mel (with the team) demonstrate how compassionate culture and cross‑sector pharmacy collaboration deliver safer, more connected and more impactful patient care.

Elizabeth Jenkins
Lead Pharmacist for Diabetes in Primary Care, ABUHB
Elizabeth Jenkins qualified as a pharmacist in 2003 after completing her MPharm at the University of Portsmouth. She began her career in community pharmacy, where she worked for 14 years before transitioning into primary care. During this time, she completed a postgraduate diploma in therapeutics and qualified as an independent prescriber, developing a specialist focus in the management of patients with type 2 diabetes.
Over the past three years, Elizabeth has led a large‑scale cardiorenal optimisation project across Aneurin Bevan University Health Board (ABUHB). The demonstrated success of this work directly contributed to the implementation of chronic kidney disease (CKD) optimisation within the General Medical Services (GMS) contract, influencing national policy and investment in Wales.
Elizabeth recently completed a postgraduate diploma in renal medicine and is now the Lead Pharmacist for Diabetes in Primary Care within ABUHB. She combines clinical expertise with system‑level leadership to drive evidence‑based, cost‑effective improvements in diabetes, renal, and cardiovascular care.
