Description:
Personalised care means people have choice and control over the way their care is planned and delivered, based on ‘what matters’ to them and their individual strengths and needs. This represents a new relationship between people, professionals and the health and care system. Personalised care provides a positive shift in power and decision making that enables people to feel informed, have a voice, to be heard and be connected to each other and their communities.
Motivational Interviewing is a powerful technique which empowers the patient about their healthcare and aids shared decision making. You will learn how to use this as part of your day to day practice and help with polypharmacy reviews.
Who can attend:
Anyone that works with Polypharmacy in their day-to-day practice, from any sector, including Pharmacy Technicians.
We will engage in an open discussion regarding the challenges encountered across various sectors. Through collaboration, we will come to understand that, despite differing contexts, our similarities outweigh our differences. Most significantly, we already possess the ability to effect change together.

- To ensure we capture learning from across the board and constantly improve
- To act as a receptacle of knowledge and ideas that we can write up and share with others
- To become the go to group for Polypharmacy and deprescribing in the West Midlands
- To become an open forum: a safe space for open discussion, debate, challenge and celebrate success

We must also emphasise that this will be the final West Midlands Polypharmacy Community of Practice hosted by Health Innovation West Midlands. These sessions will continue beyond January with a new host organisation.
