The content of the workshop will provide an update for primary care teams on aspects of the whole heart failure pathway, including case finding, diagnosis and treatment recommendations, and the national drivers that will enable PCNs to optimise care.
At the end of this workshop, attendees will:
- Have an awareness of the recommended heart failure pathway of care
- Understand why heart failure is important in the wider context of cardiovascular disease prevention
- Understand the evidence-based therapies available for heart failure management
- Understand the importance of case finding to support the identification of people who will benefit from heart failure therapies and/or optimisation.
