Living with heart failure is becoming increasingly common. In the UK, heart failure affects approximately 920,000 people with 200,000 new cases annually, and these numbers are increasing. It is now estimated that diagnosed cases are predicted to nearly double by 2040.

The webinar will showcase projects that have implemented approaches to rapidly optimise heart failure patient treatment leading to reduced symptoms, improved quality of life, and reducing the risk of mortality or heart failure readmission compared with usual care.
