This is a 2 day course on 26th February and 5th March. Please make sure you are available to attend both days.

These days will focus on a holistic approach to cardiovascular disease areas, including Coronary Heart Disease (CHD), Stroke, Heart Failure, and Hypertension, equipping practitioners with the skills and knowledge necessary to conduct thorough annual reviews.

Learning Outcomes:

  • Explain the basic physiology of cardiovascular disease.
  • Be able to perform risk assessment with patients and discuss modifiable factors such as diet and exercise.
  • Explain the targets for blood pressure and cholesterol and how these affect primary and secondary prevention of CVD.
  • Describe the basic anti-hypertensive and cholesterol-lowering medications involved in CVD.
  • Name the major complications that may arise in people with a long duration of CVD and measures that may limit or prevent them.
  • Describe the key advice to patients surrounding Heart Failure and recognition of when to escalate/refer.
  • Be able to perform a safe review for stroke/CHD/HF recognising how to work within your professional limits and when to signpost.
  • Provide examples of referral pathways to other services e.g., weight management, secondary care, activity, and psychological services etc
  • Explain the key components and process of an annual review and a self-management plan.
