The presentation will discuss a large-scale NIHR Research Project studying medication management in older people on polypharmacy living in their own homes. 

What we will cover:

    • Key aspects of medication optimisation such as adherence.

    • The patient view on medication optimisation.

    • A key target for clinical intervention by pharmacy staff and others.

Anyone that works with Polypharmacy in their day-to-day practice, from any sector, including pharmacy technicians.

We will talk honestly around the challenges we face in different sectors and through collaboration we will realise that whilst the setting may differ; we have more similarities than differences and most importantly we already hold the capability to create change between us.
