Heart Failure Content

The content of the workshop will provide an update for primary care teams on aspects of the whole heart failure pathway, including case finding, diagnosis and treatment recommendations, and the national drivers that will enable PCNS to optimise care.

At the end of this heart failure workshop, attendees will:

  • have an awareness of the recommended heart failure pathway of care.
  • have an understanding of why heart failure is important in the wider context of Cardiovascular Disease prevention and in relation to CORE20Plus5.
  • have an understanding of the novel therapies available for heart failure management.
  • understand the importance of case finding to support the identification of people who will benefit from heart failure therapies and/or optimisation.

Learn about how the Myton Hospice services can support your patients (18yrs+) and their loved ones from the point of a diagnosis to end of life. Myton Hospice’s outpatient services can support people with a prognosis of years to months, to improve their quality of life, wellbeing and to improve their end of life care outcomes.

The session aims to equip you with the knowledge and tools to identify patients who would benefit from accessing support from Myton’s outpatients services and how you can directly refer your patients into the variety of services that are available.
